LMG	PROJECT	FUNDED	DESCRIPTION
Mangakino	Stepping Out in Mangakino	SIA	Funded pedometers for overweight/obese and/or diagnosed diabetics with Mangakino Health Centre. Programme delivered by trained practice nurses including motivational assessment and both telephone and face to face follow up
Mangakino	INR Desktop	SIA	This project funded a nearpatient testing machine based at Mangakino Health Centre that gives a rapid INR reading. INR measurements are usually taken to ensure warfarin, a blood thinner is at the correct dose. Funding for 17 patients in first round.
Mangakino	Fit Gym	HP (and TDC funding)	Seed funding to Sports and Leisure Association Mangakino (SLAM) to establish a community gymnasium and employ a coordinator who assists delivering youth and young parents health promotion messages in line with the PHOs strategic priorities for youth
Turangi	Nga Tamariki O Turangi – Oral Health	SIA	One FTE (0.5 assistant/0.5 educator) to assist the dental therapist in Turangi. Contract with LDHB as employing organisation
Turangi	Home Insulation	SIA (and a range of other funders)	A project to retrofit 100 homes in Turangi through Energy Options. LDHB, MoH, EECA, Turangi/ Tongariro Community Board and PHO are jointly funding the project. The project is still going and at last report had completed 144 homes
Turangi	Waka Ama	HP	Seed funding to Turangi Water Sports Committee and Nga Toka Toru Charitable Trust to purchase a waka and deliver health promotion messages through runanga in line with the PHOs strategic priorities for youth. This project is supported by THSL through their health promotion contracts for physical activity and youth/rangitahi
Taupo	Pacific Island Health	SIA	A project to support Lakes Pasifika to establish the Trust and run three core sub projects: improving the rate of enrolment, assisting with the social determinants of health including housing and welfare benefits, and promoting physical activity and nutrition. Employment of a 1 FTE Community Health Worker, payment to general practice for extended health and wellness checks and a contract for Langauge Line Translation Service also part of the project.
Taupo	Oral Health	SIA	1.6 FTE (0.8 assistant/0.8 educator) to assist the Taupo based dental therapists and to promote and educate 0-18 year olds. Contract with LDHB as employing organisation
Taupo	Nutrition workshops	HP (and REAP)	Jointly funded project to deliver community based nutrition seminars (50% of which are targeted at Maori and high need families). Seminars are delivered by Anne Battersby and include resources for attendees such as cookbooks.
Taupo/Turangi	Community Dietitian	SIA	A project to provide free access to dietetic services in Taupo and Turangi. The dietitian will be available to local GPs and will link with the EoP and FLC programmes.
All LMGs	u-25 Sexual health	SIA	A project offering free GP/nurse consultations to young people under 25 years of age. Evaluation report completed after first eight months, project rolled over for 06/ 07 year
All LMGs	Fit 4 School	SIA	A project to increase the uptake of the four year old immunisation as well as providing an opportunity to screen children for health issues that may adversely impact on their transition to school. Service is provided through all general practices and Tuwharetoa Health Services Limited
All LMGs	Eldercare	SIA	A project to provide free assessments in the home by trained practice nurses to selected at risk elderly patients. This is followed up by a meeting with the GP to develop an action plan personalised for the patient
